Do you provide a Letter of Medical Necessity (LMN) to help patients purchase FODZYME with HSA or FSA funds?Updated 2 months ago
Yes! Since each insurer has their own approval process on what purchases are eligible for HSA or FSA coverage, we provide a templated letter of medical necessity (LMN) for clinicians to fill out on behalf of their patients.
As background, HSA and FSA funds can be put towards purchases of products used to treat a medical diagnosis, such as IBS. Some companies are set up to accept HSA/FSA cards directly based on their merchant category, while others fall outside of the HSA/FSA accepted merchant category. We’re in the latter, which means an insurer will need to pre-approve FODZYME for reimbursement.
The process for this approval is:
-
Patients (ie. the insured) submit a letter of medical necessity (LMN) filled out by a medical provider describing how a product is used to treat a medical condition
-
The benefits manager at the insurer approves the product as eligible for reimbursement
-
Patients purchase the product and submit a receipt to their insurer for reimbursement

Note: some insurers will provide their own LMN template and prefer that providers complete their form.
